Reminder:

1. Student downloads/print the evaluation form.
2. Student hand over the form to industrial supervisor.
3. Industrial supervisor/Organization must give back the evaluation form to me by:
• Postage: Post the form to the given address and make sure evaluation form is concealed (put the evaluation form in an envelope).
• Fax: Fax to me according to given fax number. Please inform me earlier.
• Email: Email to me according to given email address.
